Especially after an injury or an episode of hospitalisation.

Comprehensive home environment assessment, prescription of assistive technology and home modifications (AT-HM).

Assessment and intervention of functional activities, e.g. showering, dressing, kitchen assessment.

In-home or telehealth for dieteitic assessment and recommendation by our accredited practising dietitians. We provide trials for different oral nutritional supplement products and prescribe appropriately.

It is a multidisciplinary program.

Starting with a comprehensive assessment with our experienced Gerontology physiotherapist or occupational therapist.

Then followed by a structured program (e.g. 2 per week for 8 weeks for improving physical capacity) and/or appropriate referrals (e.g. home modifications for reducing falls risk).

Concluded by an assessment towards the end of the program and a management plan for ongoing falls prevention.

This program is for those who recently discharged from hospital and experiencing any form of mobility and/or function lost.

Client is firstly assessed by a physiotherapist and/or occupational therapist, then commence an intensive programs.

The goal is to return to client’s previous baseline at the best of our ability.

It’s fun!

It’s community-based. Annual assessment by a physiotherapist to monitor client’s physical ability (mobility assessment, balance, strength and fitness).

Advice and recommendations provided from an allied health perspective.

The goal is to maintain as a healthy and physical able body to live the best quality of life.
